To request permission to use material from a Self-Counsel Press book or forms kit, please use the Permission Request Form. Alternatively, you may submit your request via fax at (360) 676-4549, or in writing by conventional mail.

Note: The Adobe Acrobat Reader form can be filled-in with Version 5 or later of Acrobat Reader, but you can only print the completed form; you cannot save or e-mail it because of limits in Adobe Reader software.

Please download either of the Permissions Request Forms (MS Word format in a ZIP file or Acrobat format in a ZIP file) and use the form to submit your request to:
